The doctors welcome new patients who live within our practice area. As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire.
Your named GP will be Dr Shadia Abdalla. This does not affect your right to choose your preferred clinician.

Using a paper form

You can join the practice by filling in a paper form.

You can either:

  • download and print the registration form (PDF, 199KB, 2 pages)
  • get a paper copy from the surgery

When you have completed the form, please send it to the surgery by email or drop it off at reception.

It’s helpful if you bring or include a copy of your ID. This could be:

  • passport
  • birth certificate
  • HC2 certificate
  • rough sleepers’ identity badge
  • hostel or accommodation registration or mail forwarding letter

Overseas Visitors

Those who visit the UK from overseas must pay for their care when they are in England.

In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.

Temporary Registration

If you’re currently staying with a friend or family member within our GP surgery area for more than 24 hours but less than 3 months, you can ask to register as a temporary resident. You can find more information on registering as a temporary resident on the NHS website.
